该影片的故事灵感来源于2015年轰动一时的“陆勇事件”，当时这件事情在社会生引起了巨大的轰动。据悉。陆勇检查出患有慢性白血病之后，前前后后光是医药费就花费了几十万。最后没有钱了，当时陆勇还想继续活着，之后便开始贩卖这种天价药，并且这种药也救了很多人的性命。
电影剧情：慢粒白血病病人程勇（徐峥饰）为了自救从印度带回了天价药（格列卫）的仿制药VEENAT，并私自贩卖，引起警方调查。期间，程勇在医生舒曼的帮助和指引下，从自私走向无私，为病人的生存权而抗争，最终自首投案，赢得了尊严，多位角色的命运也因为“药”这一元素串联，演绎了一场别开生面的草根众生相。从读剧本被角色魅力所吸引，到努力为观众呈现一个“大家都没见过的徐峥”，徐峥在深入体会角色特点的过程中，赋予了角色生命力。影片聚焦因“药”而展开的小人物成长为平民英雄的故事。程勇这个角色的成长过程，交织着复杂的心理变化，塑造角色的难度不言而喻，导演文牧野给予了角色“烂人”的设定，并透露徐峥在影片中有130余场高强度戏份。徐峥阐述：“这个角色最打动我的就是他内心的转变，一开始是想自救，是想赚钱，在过程中见证到了生命的脆弱，然后主动站了出来。”为了塑造出这个角色，
徐峥在片场始终保持着全情投入的状态，以体会角色心理，正如他自己形容的，“感觉自己像一个小孩子重新回到片厂”，他始终以一种热情的状态活跃在繁重的拍摄任务之中。
